CSU now has a great resource that is easy, on-line, and confidential. The Alcohol & Drug Online Assessment can be used by any student.
To use the resource:
- Go to https://www.myonlineforms.com/csu
- Enter your ename from your CSU EID, followed by your birth date using two digits for month, day, and year. (For most students, your ename is a first initial followed by last name).
- Enter the following access code: voluntary
* If you are required to complete Infosoft by the university, you will receive a different access code. - Follow instructions and complete the DAY Programs Assessment.
You will receive individualized feedback that includes specific recommendations and the option to attend a service offered by DAY (Drugs, Alcohol & You) Programs that fits your current use and level of risk.
